Privacy statement

We are committed to protecting the privacy of patient information and to handling your personal information in an ethical and responsible manner in accordance with relevant legislation and legal duty. This statement is a summary of our Practice Privacy Policy that is available for your perusal. 

We collect information about you to provide medical care and treatment and to manage our medical practice. The information includes your personal details and a full medical history. This information is stored in our electronic medical records system and/or in hand written medical records.

Your information will be used in the following ways:

  • Exchanged with organizations, doctors, nurses, therapists, medical technicians and others for the purpose of providing and administering health services; for example, Medicare, private health insurance funds, referring to other specialists and for requesting diagnostic services.  
  • We may if necessary, or when required by law, disclose your personal information to police, insurers, solicitors, government regulatory bodies, tribunals, courts of law, and hospitals.
  • For administrative and management purposes. We may disclose information about you to other contractors, such as our management software and technology providers or dept collection agent. These contractors are required not to use information for any purpose except for the activities we have agreed. They must also comply with security and confidentiality requirements. De-identified data may be used for statistical analysis, systems development and research purposes. 
  • Your information will be shared with the Mater Hospital for inclusion in the Mater Shared Electronic Health Record. Health information may also be uploaded to your My Health Record.

Unless we have obtained your written consent, your information will not be used for direct marketing, transferred to an overseas recipient or used for any other purpose. 

You are entitled to request a copy of, and correct, the stored information. If you have any questions or complaints about privacy please discuss these matters with Dr Fiona Dalzell.
